The OGS issues the following documents:
1. Certificate of Good Moral Character
2. Referral to Mental Health Agencies
3. Recommendation from Teachers and Guidance Counselors
The request may be made through e-mail or by physically accomplishing the request form at the OGS.
The OGS may be reached through:

The referee (parent, teacher or schoolmate) may complete the referral form at the OGS.
Walk-in clients are also welcome at the counseling room.
