FAU services include first aid treatment to common illnesses and minor injuries, administering prescribed medications, support in managing chronic illnesses, and providing health screenings and education.
FAU requires an endorsement letter from the parent or guardian, along with the medication in its original packaging. This letter must include the student’s name, grade, and section, the dosage and timing of administration, and any special instructions if applicable.
If the student have fever, vomited 2 or more times, diarrhea, with signs and symptoms of communicable diseases, with injury or illness that requires rest and as advised by the doctor.
These includes rashes with fever, blisters on the body, redness of the eye with itchiness or discharge, and the like.
Notify the adviser and school nurse. Before attending class, the student must present a medical certificate or letter from the parent/guardian to the school nurse for clearance purposes and absence slip.
Parents must inform the adviser and contact/visit the school nurse to discuss the health condition and restrictions of student. The parent must submit a medical certificate specifying the restrictions.
